Immunisation Hanbook 9th Edition:

HPV Register - Collecting Information and Reporting:

Victorian Cytology Services has been contracted to develop the HPV register. As this register is not yet operational, all practices will be asked to collect and hold the information until it can be forwarded on.  You can collect data by:

  1. Using your medical software -  Below are step by step instructions for collecting HPV information using Medical Director, Genie, Zedmed, Best Practice and Communicare software and examples of what the printout of data looks like. These also include instructions to help retrieve the information in a report format which can then be sent to the register later in the year.  
  1. If you do not have desktop software we still encourage you to collect the data. Below are a couple of templates that can be used to collect the data with the fields that will be needed to populate the proposed register.

 

  • Manual template - excel. This spreadsheet can be populated and kept electronically (in excel) or printed off and populated by hand and kept in hardcopy.

  • Manual template - word. This table can be populated and kept electronically (in word) or printed off and populated by hand and kept in hard copy.
All data needs to be kept safely until the proposed register is operational later this year.

With the introduction of the new Victorian Immunisation Schedule 1st March 2008, there is a new DHS vaccine order form  and resources order form which must be used from March 1st 2008.  This and all other frequently used Immunisation order forms can be downloaded from the DHS website below:   http://www.health.vic.gov.au/immunisation/general/forms

The DHS immunisation website has been updated with translated fact sheets including the new fact sheet for Infanrix hexa vaccine and the common reactions sheet at http://www.health.vic.gov.au/immunisation/language The languages that have been updated due to the immunisation schedule changes are as follows:

Arabic, Bosnian, Chinese, Greek, Indonesian, Italian, Khmer/Cambodian, Macedonian, Serbian, Sinhalese, Somali, Turkish and Vietnamese.
